At the beginning, you need to determine the maximum amount of infusion fluid that may drip out of the bottle before the replacement. For this, you need to subtract the 5 ml "safety buffer" from the 40 ml from the infusion bottle.

$40\text{ml}-5\text{ml}=35\text{ml}$

Now, determine the number of minutes until this amound of infusion has run out.

$\frac{35\text{ml}}{3\text{ml}}=\frac{35}{3}$

You may shorten the ml.

$\frac{35}{3}=11\frac{2}{3}$

The fraction in the end is converted into seconds.

$\frac{2}{3}\cdot 60\text{sec}=40\text{sec}$

So in total, you get:

$11\text{min}+40\text{sec}=11\text{min}40\text{sec}$

The infusion bottle must be replaced after 11 minutes and 40 seconds at the latest.
